Gray boxes / lines at top of form

I'm seeing gray boxes/lines at the top of my booking form which I can seem to remove or even find out why it's happening. I've tried to insert a picture and I don't see how based on the help form/instruction.

Hi Kagule, can you please share link to that page? Sounds like css overlap between EA and site theme. I will know more details once I take a look.
