If for any reason I have to deactivate and then reactivate easy appointments plugin it creates a second set of identical form fields, if I deactivate again and reactivate it creates a 3rd set of form fields. The only way to get rid of them is to uninstall plugin and reinstall plugin which then looses all appointments and settings. This also happens if I try to upgrade plugin to latest version. As there are no other similar problems on the forum I assume its to do with either my other plugins or wordpress installation. I have tried deactivating the other plugins but didnt make any difference can you suggest a way round this.  I dont think I had compatibility mode selected when this occurred but didnt want to try again as its such a long winded procedure to reinstall all appointment's etc if it happens again
