Form Below Not Showing

I posted a problem a couple of weeks ago and you said it would be fixed in the next update: https://easy-appointments.com/support/including-custom-form-fields-in-email/ I see there was an update, which I installed, but now the form to collect the info (name, email, etc) isn’t showing up. I select the employee, the date, the time, and the calendar just fades a bit and the form doesn’t load. It does this in FIrefox and Chrome. Screenshot: https://prnt.sc/12fsskw

I switched from standard to bootstrap and now it shows up. I wouldn't say it's solved, but I can deal with it now.

Can you please create page with bootstrap form so I can test it? There must be reason why form is not rendered. Best regards, Nikola

OK, now it is working with standard and bootstrap. Don't know what changed: Standard:

Thanks for update, maybe something got cached on that page. But main thing it is working now :) I will close this thread. If there are something new you can open new one. Best regards, Nikola
