Issue with Custom form fields

Not working in Avada theme to delete any custom form fields, and when Im adding fields they become more and more? It can be like 5 of the same that pops up. Started up one easy appointment on another theme and it worked, but the custom form fields looks like the default on the adminview, but on the page they got my own words?

Hi,
custom fields rely on some build in WP scripts and looks like that theme is changing that. Can you please check if you have access to the database custom meta fields table for those duplicates?
