on my website www.relaxitaxi.be you can see when you click the call to action button 'Afspraak op mijn gewenste locatie maken' you get the right form (

BUT the months of the calendar shown is in Russian (think its russian)
I dont have any settings in Russian, so my question is: how to get this in Dutch (be = Belgium I hope? Or is that Belarus you chose??.... I will change it in NL and maybe than its solved but plse check the country codes then ;-) thanks you...
