My form was working fine then all of a sudden it stopped sending the confirm or cancel buttons to the workers.  I tried adding them at the end of the admin form #link_confirm#, #link_cancel#, but it just displays a long link that when clicked does send a confirmation/cancel email to the user but not the worker or admin.  The workers are receiving the same admin form that the admin sees, it used to be blue and white boxes, now its all white regular formatted email covernashville.com
