[3.12.19] Form fail to render when location is predefined in shortcode

In version 3.12.19.
When using bootstrap short code:

Form fails to render list of Workers. Throwing error in PHP:

Removing Location tag from short code fixes the problem.
Additional error in PHP log:

Another finding. This issue is not manifesting itself when there is only one location in the DB wp_ea_locations table.
