Free version worked great, but suddenly not working anymore!

Was very happy I found it and like the way it works... worked.
Busy working on a website and after a few changes the form could not be used any longer from the front end. Template: Divi
Wordpress: latest version
PHP: latest version 7.1.16
Changes made after it stopped working:
- Google map inserted
- SSL certificate
- Sticky menu Deactivated all, but still not working.
Hope you can help me out!

Hi Hetty, I have just created Appointment via Form on your site? Are you sure that is not working?

You are right: last night I managed to get it to work again and did indeed make an appointement myself as well.
Seemed as if the Google Map embed code created a conflict. I thought at first that I had not installed my autoreplies correctly, as I did not receive any notifications.
However, I don't see any reservations/bookings in the back end either, even after refreshing.
So something is still nog right, hope to figger out what the problem is soon.
I'll keep you posted, otherwise I'll get back to you!
