Hi :)
new version of EasyAppointments is here 3.3.0 there we have added option to show both start and end time for slot in customers form.
Also we have added ids to custom form fields html.

You are right. For some reason it is not latest version. There were some changes regarding id values for custom form fields in `ea_bootstrap` shortcode.
