Plugin dissapears when selecting date

After selecting the date the entire plugin dissapears. I completed a full reset, still not working.

Waited 3 weeks for a reply on the forum or contact form, no reply – no nothing. Resetted the plugin twice, still the same. Now (week 4 1/2) we switched to a different plugin. Plugin is still on our website (still not working) – on an alternative link.
